The lottery should work like this:
1. Rank every team by pre-lottery rankings.
2. Roll a die for each team. Add that number to the team's
ranking.
3. Where there are ties, roll again for tie breakers, keeping all
the tied teams together.
Nobody moves a long ways. Being the worst team in the playoffs or
the best team out of the playoffs is about the same.
